The purpose of the Medicaid Prenatal Care Coordination program is to provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes.

Our job is not done once delivery is over. 60 days post-partum you are eligible to enroll in the childcare coordination program. Our team will stay in close contact with you after birth to provide any assistance you need for the healthy growth and development for your infant.
The Medicaid CCC benefit allows members, and when appropriate, the member's family to continue to gain additional access to providers for the coordination of services, including necessary medical, vocational, educational and other essential services required to get you and your family off to a healthy beginning.
Member's Eligibility
Members looking to enroll in CCC program must enroll their child within 8 weeks following the child's birth.
The child may be enrolled up until they reach the age of 7 years old.
The program is voluntary, members can disenroll at any time prior to the child's seventh birthday upon notice to the provider.
